*Baby born May 8th 2016, didn’t hear about it till getting served papers 2 1/2 months later
* first time seeing baby was over Skype
* Mother only allowed 10% custody for father, Tue/Thur/Fri/ 3:30-7 Sun 10-4
* Baby showed up dirty, with bruises or scratches
* Mother refuses to co parent at all
* Mother has called police on 3 exchanges and lied to police
* Mother has mental problems and refuses to seek help
*Mother was sexually abused by brother and cousin and has not had counseling
* Mother admitted to lying about restraining order
* Judge refuses to punish her for lying to the court, throwing out false accusations about father and family
* Has taken the baby to the doctor at least 30 times in his life, he’s only 1
* Has black in mold in house and has had bronchitis almost all his life
* Judge doesn’t care about babies health

The Reasonable Modification Final Rule, effective July 13, 2015, clarifies that public transportation providers are required to make reasonable modifications to their polices, practices, and procedures to avoid discrimination and ensure programs and services are accessible to individuals with disabilities. It applies to public entities providing fixed route, demand-response (dial-a-ride), and complementary paratransit services. It establishes that an individual’s disability cannot preclude a public transportation entity from providing full access to its service, except when it would fundamentally alter the service.

As the Plaintiff in the above-captioned matter, I respectfully ask the Court for permission to participate in electronic case filing (“e-filing”) in this case. I hereby affirm that:
1. I have reviewed the requirements for e-filing and WILL abide by them.
2. I understand that once I register for e-filing, I will receive notices and documents only by email.
3. I have regular access to the technical requirements necessary to e-file successfully: A computer with internet access; An email account on a daily basis to receive notifications from the Court and notices from the e-filing system. A scanner to convert documents that are only in paper format into electronic files; A printer or copier to create required paper copies such as chambers copies; A word-processing program to create documents; and A pdf reader and a pdf writer to convert word processing documents into pdf format, the only electronic format in which documents can be e-filed.
